Floral Donation Agreement
By filling out and signing the Floral Donation Agreement below, I/we agree to donate the floral materials listed above to Second Bloom Ministries, a nonprofit organization based in North Carolina. I understand and agree to the following:

I am the rightful owner or an authorized party able to donate the listed floral materials, and the materials are free from pests, disease, or hazardous substances.

I grant Second Bloom Ministries permission to collect the flowers and greenery.

I understand that the organization will repurpose the flowers into smaller arrangements and deliver them to individuals in nursing homes, hospitals, hospice care, and throughout the community. I also understand that the organization cannot guarantee the specific use, timing, or placement of the donated floral materials.

Donated materials will not be returned and may be trimmed, redesigned, or otherwise altered as needed.

I give permission to Second Bloom Ministries, Inc. to take and use photographs and/or videos of me (or of donated materials, such as floral arrangements) for ministry purposes, including social media, the website, newsletters, and promotional materials. I understand that these images will be used respectfully to share the mission of Second Bloom Ministries and that no compensation will be provided.

I understand that photos or videos of donated flowers and arrangements may also be used by Second Bloom Ministries to inspire others and share our mission.

This is a voluntary donation with no compensation expected, and donations may or may not be tax-deductible depending on IRS guidelines.

I release Second Bloom Ministries, its volunteers, and representatives from any liability or damages arising from the pickup, transport, or redistribution of donated floral materials.

My personal information will be kept confidential and used only for donation purposes and tax documentation if applicable.
